Thank you for choosing Better Health Oasis for your men’s health needs. As part of our
services, we offer telehealth visits for consultations, follow-ups, treatment monitoring,
and management of conditions such as Testosterone Replacement Therapy (TRT),
weight loss, and erectile dysfunction (ED). This Telehealth Informed Consent outlines
the benefits, limitations, and responsibilities associated with receiving medical care via
virtual platforms.
By signing this agreement, you acknowledge that you understand and consent to
participate in telehealth services.
- Purpose of Telehealth Services
Telehealth allows you to receive medical care remotely using secure audio and video
technology. Services may include:
Initial and follow-up consultations
Treatment plan reviews (TRT, weight loss, ED)
Medication management and prescription renewals
Lab result discussions
Health and lifestyle coaching
These visits are conducted via a HIPAA-compliant video platform and are intended to
supplement, not replace, in-person care when appropriate. - Benefits of Telehealth
Convenience: Access care from home or a private location.
Time-saving: Reduced travel and wait times.
Continuity of care: Easier follow-ups and monitoring.
Privacy: Secure, encrypted communication platforms. - Limitations and Risks
While telehealth is effective for many concerns, it has limitations:
Physical exams are limited. Your provider cannot perform hands-on assessments
(e.g., testicular exams, abdominal palpation) virtually.
Diagnosis may be less accurate without in-person evaluation or immediate
testing.
Technology issues (e.g., poor internet, audio/video failure) may disrupt the visit.
Emergency care cannot be provided. If you experience a medical emergency,
you must call 911 or go to the nearest emergency room.
Not all conditions are suitable for telehealth. If your provider determines an in-
person visit is needed, you will be advised accordingly. - Technology Requirements
To participate in telehealth visits, you must have:
A smartphone, tablet, or computer with a camera and microphone
A stable internet connection
Access to our secure patient portal or video platform (e.g., Zoom for Healthcare,
Doxy, or similar HIPAA-compliant service)
Basic digital literacy to navigate the platform
You are responsible for ensuring your device and environment are private and secure
during the visit. - Privacy and Security
We use HIPAA-compliant telehealth platforms that encrypt your data to protect your
privacy. However, no electronic transmission is 100% secure. You agree to:
Conduct visits in a private, confidential setting.
Avoid using public Wi-Fi or shared devices.
Not record the session without prior written consent from Better Health Oasis.
Your medical information will be stored securely in our electronic health record (EHR)
system, just as with in-person visits.
- Informed Consent and Medical Decision-Making
You will receive the same standard of care as in-person visits to the extent
possible.
Your provider will review your medical history, symptoms, and treatment goals
before making recommendations.
You have the right to ask questions, request clarification, and decline any
treatment recommendation.
Prescriptions may be issued based on clinical judgment during the virtual visit, in
compliance with state and federal laws. - Emergency and Urgent Situations
Telehealth is not appropriate for emergencies. If you experience any of the following,
seek immediate in-person medical attention:
Chest pain, shortness of breath, or signs of heart attack/stroke
Severe allergic reaction
Sudden vision changes or priapism (erection lasting more than 4 hours)
Severe dizziness, fainting, or confusion
For non-emergency urgent concerns between visits, contact our office during business
hours. - Costs and Insurance
Telehealth visit fees are the same as in-person visits unless otherwise stated.
We do not bill insurance directly for most TRT, weight loss, or ED services (often
considered wellness or elective).
Upon request, we can provide a superbill for potential out-of-network
reimbursement.
Payment is due at the time of service.
- Right to Withdraw Consent
You may discontinue telehealth services at any time and request in-person visits
instead. Simply contact our office to reschedule. - Changes to This Consent
Better Health Oasis reserves the right to update this Telehealth Consent. You will be
notified of any changes, and continued participation constitutes acceptance. - Acknowledgment and Consent
By signing below, I confirm that:
I have read and understand the risks, benefits, and limitations of telehealth.
I consent to receive medical care via telehealth from Better Health Oasis.
I understand that I can request an in-person visit at any time.
I am responsible for ensuring a private, secure environment for my visit.
This telehealth service is available to patients located in [Insert State(s)] at the time of
the visit, in compliance with state medical board regulations.
