Welcome to Better Health Oasis. By receiving medical services from our
clinic—specifically Testosterone Replacement Therapy (TRT), weight loss
management, and erectile dysfunction (ED) treatment—you agree to the following
Terms of Treatment. Please read this agreement carefully, as it outlines your
responsibilities, our medical protocols, and the expectations of the provider-patient
relationship.

1. Purpose of Agreement
   This agreement governs your participation in medical treatment programs offered by
   Better Health Oasis, including but not limited to:
    Testosterone Replacement Therapy (TRT)
    Medical Weight Loss Programs
    Erectile Dysfunction (ED) Treatment and Management
   These services are provided by licensed medical professionals and are intended for the
   diagnosis, monitoring, and treatment of medical conditions under ongoing clinical
   supervision.
2. Patient Responsibilities
   As a patient, you agree to:
    Provide complete, accurate, and truthful information about your medical history,
   current symptoms, medications, supplements, lifestyle, and any other relevant
   health details.
    Attend all scheduled appointments (initial, follow-up, lab draws, and evaluations).
    Follow prescribed treatment plans, including medication regimens, diet, exercise,
   and lifestyle modifications.
    Notify us promptly of any changes in your health, side effects, or concerns
   related to treatment.
    Undergo routine blood work and health assessments as required for safe
   monitoring.

 Maintain open and honest communication with your healthcare provider.
 Pay all fees associated with your care in a timely manner (see Financial Policy
below).
Failure to comply with these responsibilities may result in modification or discontinuation
of treatment.

3. Medical Supervision and Monitoring
   All treatments at Better Health Oasis are conducted under the supervision of licensed
   medical providers. You understand and agree that:
    TRT, weight loss medications, and ED treatments require ongoing monitoring to
   ensure safety and efficacy.
    You will be required to have regular blood work (typically every 3–6 months for
   TRT; frequency may vary for weight loss/ED) to assess hormone levels,
   liver/kidney function, lipids, PSA, and other relevant markers.
    Your treatment plan may be adjusted, paused, or discontinued based on lab
   results, clinical response, or safety concerns.
    You are not guaranteed specific results (e.g., weight loss amount, energy
   improvement, sexual performance), as outcomes depend on individual
   physiology, adherence, and other factors.
4. Risks and Informed Consent
   You acknowledge that all medical treatments carry potential risks. Your provider will
   review the specific risks associated with your treatment plan, which may include but are
   not limited to:
    TRT Risks: Increased red blood cell count (polycythemia), acne, sleep apnea,
   testicular atrophy, infertility, prostate changes, mood fluctuations.
    Weight Loss Medication Risks: Nausea, gastrointestinal issues, increased heart
   rate, dizziness, contraindications with other conditions.
    ED Treatment Risks: Headaches, flushing, vision changes, priapism (prolonged
   erection), drug interactions (especially with nitrates).
   You have had the opportunity to ask questions and understand these risks. By signing
   this agreement, you provide informed consent for treatment.
5. Treatment Continuation and Discontinuation
    Treatment will continue as long as it is medically appropriate, and you remain
   compliant with monitoring and follow-up.
    Better Health Oasis reserves the right to discontinue treatment if:
   o You miss scheduled labs or appointments without communication.
   o Lab results indicate unsafe health parameters.
   o You misuse medications or fail to follow medical advice.
   o There is evidence of substance misuse or fraudulent activity.
    If treatment is discontinued, we will provide a summary of your care and, upon
   request, assist with records transfer to another provider.
6. Medication and Prescribing Policy
    All prescriptions are issued at the discretion of the licensed provider based on
   medical necessity.
    We do not prescribe controlled substances (e.g., stimulants, opioids) for weight
   loss.
    Medications may be delivered via mail-order pharmacy or local pharmacy, as
   available.
    You are responsible for understanding medication instructions, side effects, and
   contraindications.
    Selling, sharing, or misusing prescribed medications is illegal and grounds for
   immediate termination of care.
7. Financial Responsibility
    You are responsible for all fees related to consultations, lab work, medications,
   and program enrollment.
    Payment is due at the time of service unless other arrangements are made.
    We do not bill insurance directly for TRT, weight loss, or most ED treatments
   (these are often considered elective or wellness services). However, we can
   provide superbills for potential reimbursement.
    Late payments may result in suspension of services.
   Fees may include:
    Initial consultation
    Ongoing provider visits

 Laboratory testing (draw fees and analysis)
 Medications (billed separately by pharmacy)
 Program enrollment or membership fees (if applicable)

8. Privacy and Confidentiality
   Your medical information is protected under our Privacy Policy and in compliance with
   HIPAA. We will not disclose your health information without your consent, except as
   required by law. You are encouraged to review our full Privacy Policy.
9. Communication and Availability
    Non-urgent questions should be submitted via secure patient portal, email, or
   phone during business hours.
    We strive to respond within 1–2 business days.
    For medical emergencies, call 911 or go to the nearest emergency room. This
   clinic is not a substitute for emergency care.
10. No Guarantee of Results
    While we aim to improve your health, energy, libido, body composition, and overall well-
    being, we do not guarantee specific outcomes. Results vary based on individual factors
    including age, genetics, lifestyle, adherence, and underlying health conditions.
11. Amendments to This Agreement
    Better Health Oasis reserves the right to update these Terms of Treatment with notice.
    Continued participation in care constitutes acceptance of revised terms.
12. Acknowledgment and Consent
    By continuing with services, I confirm that:
     I have read and understood the Terms of Treatment.
     I have had the opportunity to ask questions and received satisfactory answers.
     I consent to the treatments recommended by my provider at Better Health Oasis.

 I agree to fulfill my responsibilities as a patient.

legally binding guarantee of results and is subject to the laws of the State of [Michigan
and Arizona].
